I agree with Nikodemus that adding :test-not, :key and even :from-end could be useful (consider generated code, where a :from-end could save you from creating clauses in reverse order). Maybe check the ANSI CL spec for other kinds of typical keywords that could be useful here. That's all I have to say, didn't read the other comments in much detail.
