As from [1], I think we need Johan Bockgård's agreement as well. Moreover, the file was modified by Helmut Heller and Douglas Crosher. I think we also need to add the copyright statement (or at least a note about its authors).

All of my contributions to SLIME are placed in the Public Domain. Regards Douglas Crosher
