I think that's up to you. Google maps seems to indicate that Xeros PARC is still at that address, but maybe the CommonLoops Coordinator no longer exists. Personally, I wouldn't add any additional text. It already says "request", and adding more text to interpret someone else's license seems fuzzy to me.
